A combination of many things:

Worldwide recognition, fame, etc.
Business and financial power
Political power
Cultural influence, arts, media, etc.
International connections, transport links
Universities and other institutions
Large population
Diverse range of peoples, cultures and religions
High quality architecture, notable landmarks, historical areas, parks, etc.
Sports and other events

True that Milan doesn't have any high-rises but it has a strong influence in terms of culture, finance and industry.

Well first a difference has to be made between "one of the world's greatest cities" and a "world city". The first criteria is subjective, any city can be one of the world's greatest for an individual and yet not be a world city at the same time.

As an example, for me Lima is much more of one of the world's greatest cities (my own personnal opinion, I am not making a general statement) than Frankfurt, yet it is much less of a world city.

Tokyo lacks great architecture (the buildings are covered with neonsogns so i guess that makes it better) and landmarks but itīs still a world-city.

And please don't say thats now what i aid because you responded to someone questioning nottinghams world's greatest status by posting pictures of trams and buses. Clearly you don't understand what makes a city great in the world. There has to be many different categories. Business, entertainment, facilities, world fame, size, and many more. Not just tourism. The only city in the UK that is clearly one of the world's greatest is London, the rest are just nice cities but cities like them can be found all over the world, nothing special.

Well many cities have something that's special. But few cities have many things that make it special and since there are few cities that have that, it becomes a special category of its own. And in that sense, Oxford and Cambridge aren't the same special as London, Paris, or Tokyo.

It's a bit like asking what makes of a painting a masterpiece. Mainly this comes from the recognition of the mainstream opinion about it being considered (and accepted) as such.

Two words: Influence and Culture. Pretty much everything that has been mentioned so far (political power, business, media, tourism, etc.) fits into one or both of these two umbrella terms.

A great city, must not realy be great in size, of course it shouldn't be tiny, but some 250'000 population is enough if theres a good aglomeration around.

mos important for me are:
good public transport,
a lot of cultural sights,
a lot of sporting possibilietes,
great nichgtlife,
a lot of green areas,
quiet places to relax,
must be safe,
good shopping possibilities,
lots of events,
international connections,
highstanding finance and business,
must be multi cultural...

there are some very "great" cities around the world!

Though history can help a city, it doesn't have to be a requirement. For example baghdad has an incredible history yet no one today would claim it to be one of the greatest. If a city can match the quality of other great cities in the world, than i don't see why it cannot be considered a great city.

I said influence and culture before, and a city with no history certainly wouldn't have very much culture. History is one of the most important aspects of culture. And I'm not exactly sure how a city built by Disney would be very influential outside of the tourism industry. Disney World is already kinda like its own country anyways.
