It is the oldest continuously inhabited European settlement in the Americas, and was the first seat of Spanish colonial rule in the New World , now with a metro population of over 2,000,000 its the largest city in the Caribbean country of Dominican Republic, and the seventh largest in North America.

While Santiago Founded in 1495 during the first wave of European colonization of the New World, today Santiago de los Caballeros (the first "Santiago" of the Americas) is the second largest city(after Santo Domingo) in the Dominican Republic located in the North-central region of the Republic known as Cibao valley.

Yes, the Dominican Republic is on an island in the Caribbean and the Caribbean is part of the American continent because the islands are still within the continental shelf. That's why the Caribbean islands are continental islands rather than oceanic islands. Oceanic islands don't belong to a continent because they are not on a continental shelf.

The that's why Great Britain and Ireland are part of Europe, despite being islands.

Or Japan, Sri Lanka, and Singapore being part of Asia despite being islands.

However, islands like Hawaii or Bermuda or Tahiti are not part of any continent because they are not on a continental shelf. They are oceanic islands.

The islands of the Caribbean are not part of continental North America, only Canada, Mexico, and the countries in the Central American isthmus are on continental North America. But, the islands of the Caribbean are part of the North American continent because the islands are still within the continental shelf of North America, not outside of it.
