They could just be shit decisions. Something which happens at councils across the land, thanks to the fact that decisions are made by committee, and highly paid execs with no connection to the local area put through decisions that they think might be for the "greater good" but clearly aren't. The 60s ring road decisions based on the 60s idea that everyone will drive everwhere, versus modern day pedestrianisation.
Or even the fact that some decisions need buy-in from various different councils. Why's getting the buses across Leicestershire to run on time and well so difficult? Because there's umpteen different companies and about three or four different councils involved in each route, meaning no joined up thinking ever occurs.
