Communication

Have you ever had the situation, when something in project was wrong because of problems with communication? I'd like to know something about barriers in communication between designers. (Designers and contractors/investors.) How was it possible:

IMO communication during the design is a real problem - especially in terms of marking corrections. It is time consuming (these processes are usually held during the meetings) and imprecise (telephone consultation). Isn't true?

How to improve it?
