Based on my experience I'd say:
Best: Porto (Portugal)
Worst: São Paulo Guarulhos INTL (Brazil)

I've had bad luck in O'Hare and Charlotte-Douglas, getting in and out on time. I really like Washington-Dulles and San Francisco. Maui has by far been the friendliest. Unfriendliest, O'Hare.

Best
Singapore Changi
Kuala Lumpur
Calgary
Koh Samui

Worst
JFK
Philadelphia
Heathrow (sorry but even T5 isn't enough to redeem the rest and the really poor service)
São Paulo Guarulhos

Bergamo airport is the worst I've used. I was there airside for well over an hour, and there wasn't anywhere to buy any refreshments. The only places open were a duty free shop and a souvenir shop.

They had a chaotic boarding policy too. You had to be bussed to the plane, and they invited people with kids, OAPs etc first, as they usually do. But that meant that when the bus arrived at the plane, those "priority" people who had got on first were actually last off the bus. Being with Ryanair with unreserved seating, this caused families to be split up for the flight.

The cabin staff then moaned - actually complained over the PA - that people were taking too long to find seats.

As for best, Ko Samui is certainly a pleasure for being so different, although it wouldn't be impossible for a big city airport.

I like Heathrow's Terminal 5, although much of that is because it contrasts with the usual low expectations of Heathrow.

Hong Kong and Singapore airports are really good, and I liked Bangkok's Suvarnabhumi too, which must be even better now the airport link is complete.

Worst is Philadelphia for me.

those are euro-rules and Frankfurts migration officers stick to them.
Not very intelligent rules if they had a least 200 people sent back to their point of origin...

Best I've used: Might be between PTY (constant renovation) and HAV (bit cramped but it really does the job)

Worst I've used: MIA (maybe because i disliked the city)

Are you basing your opinion on the architectural look or how well it works as an airport? I admit BWI is not much to look at from the air side, but on the ground side it is quite modern-looking, even though the space-frame design dates back to the '70's, and has been much copied (which accounts for your comment "everything looks like Baltimore). It is one of the easiest to use as far as catching a flight.

Worst in North America must be Baltimore in Maryland followed by Dallas Ft Worth.
As far as pure utilitarianism is concerned, DFW works fairly well now that some alterations have been done to it. I love the skytrain, because it goes in a loop and stops nearer to actual gates and waiting areas than the central spine type arrangement with one stop per concourse other airports tend to have.

But yeah, oh god DFW is freaking hideous inside. And on the outside too, from skylink you can see how dilapidated parts of it are.

Maybe it's the best worst then?

My rating goes as such:

1 huge terminal = good
multiple terminals = bad

Arlanda (stockholm) is a very nice, well run airport. LHR T5 is nice, but the other terminals could use a nice touching up. Also the old Berlin airport was pretty bad, luckily they tore it down.

Only nice US airport ive been to is Dulles. Baltimore,Tampa, Orlando, Newark, JFK, and Atlanta are all dreary, partly because of bad service and terrible security.
