Though it's not impressive by any stretch, the longest distance between exits that I've driven personally is on a stretch of I-81 in Montgomery County, Virginia. The Virginia Tech exit is 118, and the next exit to the north is 10 miles away (numbered 128).

If you're looking for density, try the N2 between the Cape Town city centre and the Raapenberg Road interchange (the Pinelands exit). There's 10 numbered exits in around 8.5 km of road; the actual figure is even higher as there's a few multi-exit interchanges in there as well. List of interchanges include the N1/N2 split, the M3 merging with the N2 for a few hundred metres before splitting off again, and the M5 freeway.

That stretch of road is restricted to 80 km/h for very good reasons.

On the other hand, the freeway sections of the N1 and N3 in the northern Free State have really low exit densities - the N3 toll road between Warden and Villers has four exits (including one each at either extremity) in just short of 100 km.

This includes two cloverleaves virtually on top of each other Google Maps.

This is a result of it being an early freeway (no later than 1962) and being cut through an existing road network where many main roads intersect with it.

Newcastle's Central Motorway has 4 exits in a mile (the length of this really short urban motorway).

Other motorways in the UK with high densities are urban. these include Glasgow's M8, Leed's A58(M)/A64(M) and Manchester's A57(M).

With the exception of the M8 these other motorways are typically less than two miles and are very substandard with short slip roads, no shoulders and consequently low speed limits. In fact there are many non-motorway urban routes that are similar in characteristics: A38 through Birmingham City Centre, many of the New Town Expressways etc etc.

The Dan Ryan expressway in Chicago has a stretch with a very high exit density just west of the Loop. It's one exit per block:

That must be very exciting to drive through. (Haven't been there, but I know about it.) I believe current Interstate standards would prevent such a thing being built today, and that there's talk of cleaning it up.
