200 mm seems very low, 1100 seems rather high. How come there's not one unified height somewhere in the middle?

1100mm is high, but nice for single-level trains, got a few like that in Sweden too
200mm is crazy low though. Even for doubledeckers. I don't see the need at all for the high speed lines. I predict a change in that standard within a coule of decades.

There is, it is used in some ares where high floor and low floor suburban services overlaps (Kyiv, for example), but it isn't defined officially in building codes

How much demand is there for this line? For 10 (?) years now, no direct train is running anymore on from L'viv via Rava Ruska (Gr.) to Warsaw...

I believe that it's no more than a wishful thinking, especially that article indicate about "ultra high speed line" that should indicate construction of a new dedicated HS, I guess.. That just can't be justified in a moment.

I think until 2011 there were up to two daily train connections between Lvov and Warsaw.
1. Lvov - Berlin through car via Jagodin, Warsaw (up to 2/weekly); cancelled in September 2011
2. Odessa - Warsaw through cars via Lvov, Przemysl (daily), cancelled in December 2011
