How can the majority of those failures be real? Surely even the dumbest person realize that something is up when the top of the escalator touch the ceiling?

Usually a case of a bad drawing or some communication error. And the contractor would rather build it, get paid to take it down, and get paid again to rebuild the fixed version, than to complain there and then and slow down work while the drawings are fixed.

And of course, some are taken midway through construction/refurbishment, or photoshops, or borderline vandalism for the sake of the picture. Such as the "bike lane" painted along the edge of the canal, or "let's move the slide so it points at the tree!" a bit down the list.
