Afaik all 100 stations of the Stockholm subway have elevators.

In São Paulo, the combined network of subway and suburban trains comprises 143 stations, of which only 63 stations are wheelchair accessible.

Washington Metro is 100% accessible, so it has elevators and escalators in all stations.

Let me count it...ok,on line 1 there is none,and...well,there is none on line 2,and there's not a a single one on line 3 either. Line 4 will have elevators though.

In Copenhagen all 24 metro stations have elevator, and all new planned metro stations will have two elevators each. But the Copenhagen metro is also rather new.

The S-train (S-Bahn) of Copenhagen is older, but also they have either elevators at all station or acces at grade or by ramp, so they is acces for all.

All 48 Stations on Vancouver's SkyTrain system has elevators, which wasn't the case a few years ago. New SkyTrain extensions and lines planned after the original Expo Line portion were required to have full accessibility. Granville Stn had installed a new elevator with its new entrance upon the completion of the retail/residential development above. Aside from a few entrances on different stations, all of them have elevators (most entrances that don't have an elevator is close to another entrance with an elevator).

All stations not at-grade on DART's rail system in Dallas have elevators. Cityplace Station also has 2 incline elevators:
