Queens Blvd, NYC. This is half of it. The eastbound side is on the other side of the viaduct which carries the subway. Crossing this monster is a nightmare, especially around Continental Ave, where it's 12 lane across.

In Paris the widest Boulevard (or Avenue) is the Avenue Foch : wide 120m, length 1300m. The wide of the Champs Elysees is 70m (length 1970m).

Avenida 9 de Julio has 17 lanes. Gardiner Highway in Toronto, Ontario, is even broader, it has 21 lanes.
