That's not a fake. Is a development made by two students of HAWK high school in Hildesheim: they had an idea, and made a prototype.
That's far away from what a "fake" is.

In Sweden we have lights on each side and overhead one over each lane (see pictur)in some cities and we always have a green arrow when we are allowed to turn right on red.

Green meaning it's safe to cross and the flashing amber meaning you can expect other traffic but are allowed to cross at your own risk. Never became very popular though.
These are common in certain parts of Groningen. Should be implemented more if you ask me.

There are two major types of traffic lights in Slovakia:

with arrows - offering absolute right of way in direction arrows displaying to (obviously you have to give way to ambulance, fire-fighters, police etc.). Sometimes, the amber light with a symbol of pedestrian may be lit on while turning on right (so you have to give way to pedestrians)



without arrows - you can turn wherever vertical traffic signage allows you to, but you have to give way according to general traffic rules - e.g. right of way for cars from right, giving way while turning left, giving way to pedestrians etc.



if the green light with arrow is attached to such traffic light and is also lit on, you can go in direction arrows directing to, despite of red light on major traffic light, but you have to give way to everyone else.



if the green light with left arrow is attached on opposite corner of intersection and is lit on as well, you have the certainty, the opposite direction has red light on, so you don't have to be worry giving way to vehicles from there.



And at the end - small FAIL

That's not true. I lived in Southern California and traveled throughout in more than 15 years. Some intersections have one or two signals.

Overhead traffic signals are rare in Sweden though. I never seen them anywhere in Stockholmslän.

Overhead signals are widespread here in Brisbane and south east Queensland, not sure about the rest of Australia though. There's signals at the stop line, signals on the opposite side of the junction, and signals overhead. Here's a good example of a junction;

I travel extensively in Southern California: I have not see a single mast-arm with 4 signals on it except for that one picture posted of an intersection in the Bay Area, Santa Clara to be exact. The standard is usually 1 signal, 2 if there's a protected left-turn signal. Sometimes there will be 3 if the road is wide enough, with 3 or 4 lanes in each direction. Examples of that include Beach Blvd in Orange County, Hawthorne Blvd in the South Bay, and Santa Monica Blvd in West LA.

The common way to post 2 protected left-turn signals at an intersection in California is to have a separate mast arm posted in the median just for those two signals. Here's a perfect example: Beach Blvd at Edinger Avenue in Huntington Beach.
