The N65 Tilburg - Den Bosch (The Netherlands) immediately came to mind, but I can't seem to find the SV-location. Am I looking in the right direction?

The first one is probably S7 in Poland, south of Bialobrzegi.

It is also visually similar to N51 (future A34) in France, north of Reims, but the markings and shoulders don't match.

That's right. It's a parking at the Colon in Valencia, in where left-hand driving is obligatory. Very, very weird, I can tell from my own experience.

Not so weird... I have some spaces in a carpark (an "private" one, realy big, +250 spaces) where in the entrace ramp you must drive on left. The same thing for the garage where my brother park or my gf carpark in her building.

It's a common thing when the entry ramp from the street it's the same for exit, and there's a justification... the door opening system.

As usual, we use the remote control, or in addition a "manual" electronic system with a normal key, RFID tag, magnetic plastic key or magnetic card... and these systems are placed on the wall, on the left for allow the driver use these systems... like this example...



EDIT: Where the ramp has two lanes and can't be possible to place the "manual" control in the middle because in spain you need more than 2,5~3,5m of width for each lane in a parking ramp (the legal width is not the same for all councills... ), not for one lane used for enter and leave the parking...

Did you know that large parts of the A10 south of Paris now has an uninterrupted hard shoulder?
