BR-116 widening to 3+2, near Miracatu. Expected opening early 2017. This is probably the most severe bottleneck in the Brazilian trunk highway network.

@Ful Muniz: pela foto, a pista velha é a que tem 3 pistas. Aparentemente, 1+2. Provavelmente 1 pista descida e 2 subida.

Assim sendo, acredito que a estrada com 3 pistas (velha) será usada para a subida (mais espaço para ultrapassar caminhões penando pra subir a serra) e a pista nova serão 2 pistas, usadas para descida.

The new road, in the section showed in the picture, will be used downhill. The old one, uphill. But mountain up, near the end of the Serra do Cafezal, this scheme is the opposite, the existing road will be used downhill.

Another major bottleneck in brazilian highways is the Serra das Araras in BR-116 in Rio de Janeiro state (Via Dutra). Extremely dangerous stretch in the highway between Rio and Sao Paulo.

The missing link of Campinas (informal) ring road, linking Anhanguera and Bandeirantes highways, improving the access to Viracopos Airport. Let's wait for the Google Maps to update it.

BTW, on October I travelled through Bandeirantes Highway. What a masterpiece! 5 lanes in each direction, cutting through the wooded hills. Amazing! São Paulo has indeed a first world infrastructure.
