Ok, I know there are some exceptions for these unsafe elements in the Interstate System. The bothering part in the duplicated federal brazilian highways is that U-turns are quite common in order to save just a few money (compared to the whole work), and they are there for the convenience of the real estate on the edge of the highway.

Take a look in this road http://maps.google.com.br/?ll=-27.25...k&z=15&vpsrc=6 going south, you will see 4 U-turns in a matter of 12km. With access control, the need for the U-turns decreases, and more important, the shelf life of the highway increases significantly.

Emergency U-Turns occur only where medians are widest...but they are hardly as common as those in Brazil. But like I said, it all stems from access control. Get that right, and everything else follows.
