1) Does this list take into account Low-cost terminals in certain airports ? if yes, the list is unfair.
2) How about smaller airports ? I can name many smaller airports I enjoyed using more than Incheon, Changi, HK, KL, etc.

The methodology seems okay. Yet again there can be some bias, for example, a service that might be considered to be great by a south korean, might be just okay for an european, i.e. different societies might be used to different standards, so what is a 5 over 10 for an european in the questionnaire, might be a 7 for someone else. Good news is that Zurich has climbed up to the 4th position worldwide, maybe next year... :P
