Scaffolding Collapse

Does anyone know any buildings in Melbourne where there has been a scaffolding collapse? I'm doing a report where we have to do an accident investigation. I'm going to use a scaffolding collapse as my accident. I'm thinking scaffolding was up arounding the building. It was being demolished with the facade to be preserved. Any ideas? Does anyone know of any cases where scaffolding collapses has occurred in Melbourne?

If a building is being demolished but the facade is being kept then what are the possible reasons for scaffolding to collapse? Could it only be buckling or could there be other causes?
One happened here this year with an office building which is currently being built. Haven't heard of any in Melb but I'm sure it has happened.

High winds are also known to collapse scaffolding. Do a search on BBC News and you will see a number of stories pop up.
